Registered Nurse (PRN) - Bridging Life Hospice

Location: Westminster, MD and Baltimore County and City, MD

PRN - Minimum of 4 shifts per month

Bridging Life Hospice, part of LifeBridge Health, is seeking a compassionate and skilled Registered Nurse (PRN) to join our dedicated team. As a vital part of our hospice care program, you will provide meaningful care and comfort to patients and families during the final stages of life.

Key Responsibilities:

• Conduct comprehensive and continuous assessments of patients' health status.
• Utilize family, caregivers, and medical records to guide patient assessments.
• Develop, implement, and document patient care plans, interventions, and outcomes.
• Provide patient care in accordance with hospice standards and agency policy.
• Communicate effectively with interdisciplinary team members, patients, and families.
• Participate in physician rounds and supervise Hospice Aides (HHAs).
• Offer emotional and educational support to patients and families.
• Document and update care plans based on ongoing evaluations.
• Ensure safety in the work environment and advocate for patient needs.
• Participate in team meetings, orientations, and community outreach.
• Maintain compliance with all regulatory documentation and performance improvement initiatives.
  
  

✅ Qualifications

• Registered Nurse license (RN) in good standing with the Maryland Board of Nursing (MBON).
• American Heart Association BLS certification required.
• Minimum of two years of acute care experience strongly preferred.
• Strong clinical judgment, communication, and organizational skills.

• Minimum Commitment: At least 4 shifts per month
